re:- thanks for link. Actually i recognised the website as soon as I saw it. I gave up on it last time as all i could see was a means of downloadind the form, printing it out, filling it in and putting it in the post.

You could try copying, pasting to word, fill it in (highlite boxes and replace with X) save then attach to email via contact us.
I've just done a dry run without sending. Should be OK.
